Current Employees - Benefits
Effective January 1, 2010, the medical plan for the majority of our employees is administered by UMR. This is a PPO plan using the United Healthcare Options PPO network, with an HRA account. All covered medical expenses go towards your deductible and Herzog pays the first half of that deductible (this is the HRA). The UMR website, www.umr.com, allows you to review clams, find a provider, order new ID cards and much more. You can also contact UMR at 1-877-233-1800.
The dental plan and vision plan for most of our employees is through Guardian, www.GuardianAnytime.com. You can also access information through their toll free number, 1-800-541-7846.
For those employees who are enrolled in the Herzog 401k Plan, you can access your account information online at www.massmutual.com/retire or 1-800-743-5274.
Please remember, open enrollment for all benefits is in December for a January 1 effective date. That is the one time a year that you can make changes to your benefits. You can also make a change if you have a “Qualifying Event” such as marriage, birth of a child, loss of other coverage, etc.
The 401k Plan has two entry dates per year – January 1 and July 1.
